As a user, I want the assistant to ignore sensitive or irrelevant files so that the review focuses on meaningful code changes.
Acceptance criteria:
Sensitive files such as .env are ignored.
Lock files and generated files are ignored.
Ignored files are not reviewed by the assistant.
The review result can indicate which files were skipped.
